Attention Parents of CHMS 7th or 8th grade students who are interested in playing sports this 15/16 School Year! Please join us on MONDAY August 10th for one of the evening sessions: 5:30-6:30 or 6:30- 7:30 in the Chasco Middle School Cafeteria to hear from Mr. Marrotta, our CHMS Athletic Director and to receive help in filling out the athletic forms required for participation in sports. We will provide you with the 7 documents that are required by the district and state along with having a notary present. You can also find that forms here or in our main office if you would like to access the forms prior to the meeting.
Please ensure you provide the physical form to your child’s physician to fill out and sign off on when taking your child for their annual physical. Thank you, Chargers!
