New to Pasco County Schools Registration

 

Welcome to Chasco Middle School! If you are transferring from another county or state, you are in the right place. With increased safety and well being of our fellow Charger families as a top priority, new families are able to download the registration forms below and email them when completed to mamormil@pasco.k12.fl.us. Please complete the packet and return with the required documentation. PASCO IN COUNTY STUDENT TRANSFERS

  • Proof of Residency- Any 3 of the following: water or electric bill (dated within the last 30 days), lease or mortgage paperwork, driver’s license, voter registration, or auto registration with current address.
  • Tdap booster required for incoming 7th grade
  • In county registration enrollment packet (completed in full)
 

OUT OF COUNTY/STATE & FIRST TIME FLORIDA STUDENTS

 

Students entering a Pasco County Public School for the first time at any grade level MUST present the following items upon registration:

  • Proof of Residency- Any 3 of the following: water or electric bill (dated within the last 30 days), lease or mortgage paperwork, driver’s license, voter registration, or auto registration with current address.
  • Current immunization record on a State of Florida Immunization Record 680 form (Tdap booster required for incoming 7th grade)
  • Proof of a physical exam dated & signed by a physician within the last 12 months
  • Copy of a birth certificate
  • Copy of a social security card
  • Copy of report cards, transcripts, or academic history (please include records for all grades completed (6, 7 & 8) or transfer grades if applicable
  • Special education records (if applicable) to include copies of current IEP or 504 plan
  • Legal guardianship records (if applicable)
  • Out of county registration enrollment packet (completed in full)
